Bark Boutique and Grooming Salon Service Agreement This Agreement is entered into between Bark Boutique and Grooming Salon (the “Pet Groomer”) and the client booking the service (the “Pet Owner”). By booking an appointment, the Pet Owner agrees to the following terms: PET GROOMING SERVICES The Pet Groomer agrees to provide professional grooming services, including: Bath, Haircut/Trim, and Nail Trim Basic teeth cleaning and Ear cleaning/plucking Gland expression (unless requested otherwise) Brush, blow out, styling, and finishing touches Photo Release: Pet Owner grants permission to use photos of the pet taken during the appointment for marketing and social media purposes. PET OWNER RESPONSIBILITIES The Pet Owner certifies the pet is in good health and has been free from contagious diseases for 14 days. Pet Owner must disclose all previous behavioral issues. Failure to do so makes the Pet Owner responsible for any medical expenses or property damage resulting from the pet's behavior. RABIES & VACCINATIONS By law, all pets must be up to date on vaccinations. Puppies: Minimum of Rabies and all Parvo shots. Adult Dogs: Minimum of a Rabies shot. A copy of vaccination records must be provided (via digital upload or physical copy) before service. CANCELLATIONS & LATE ARRIVALS Notice: At least 23 hours notice is required to cancel or alter an appointment. Late cancellations incur a fee of 50% of the grooming service, payable before a new appointment can be booked. Late Arrivals: If a Pet Owner arrives more than 30 minutes late, the Pet Groomer reserves the right to cancel or reschedule the appointment. PAYMENT & PICK-UP Payment is due in full at the time of pick-up. We accept Cash, Check, Visa, Mastercard, and American Express. Hard Cutoff: All pets must be picked up before 5:30 PM. Late Pick-up Fee: Pets left after 5:30 PM will incur a late fee of $1.00 per minute. If the pet is not picked up by the time the facility closes, additional overnight fees may apply. Returned Checks: A $25 fee applies to all returned checks. MATTING, FLEAS, AND TICKS Matting: Severe matting may require shearing. The Pet Groomer is not responsible for skin irritation or nicks resulting from grooming a matted coat. Parasites: If fleas/ticks are found, a mandatory $15 treatment bath will be applied, or the pet must be picked up immediately. EMERGENCIES & ACCIDENTS Grooming involves sharp tools and moving animals; while we take every precaution, accidents can occur. In a medical emergency, we will attempt to contact you immediately. If unreachable, you authorize the Pet Groomer to seek necessary veterinary care at the Pet Owner's expense. DANGEROUSLY AGGRESSIVE ANIMALS We reserve the right to stop service if an animal poses a risk to itself or staff. If a staff member or another animal is injured due to undisclosed aggression, the Pet Owner is liable for all resulting medical/veterinary expenses.
