The contract (the “Agreement”) is entered between Bark Boutique and Grooming Salon (hereinafter referred to as the “Pet Groomer” or “Pet Grooming Company”) and ________________ (hereinafter referred to as the “Pet Owner,” “You,” or “Your”) on _________________________ (the “Effective Date”) PET GROOMING SERVICES The Pet Groomer agrees to provide the following services in a professional manner: *Bath *Haircut/trim *Nail Trin *Basic teeth cleaning *Ear cleaning/plucking (unless Pet Owner requests no ear cleaning/plucking) *Gland expression (unless Pet Owner requests no gland expression) *Brush and blow out *Styling and finishing touches and Facebook Pictures Any additional services, including but not limited to flea/tick treatment, de-matting/sticker removal, de-shedding (basic/extra), matter/pelted fur, aggressive and/or difficult dogs, will be charged an extra fee. The Pet Owner can check and discuss this pricing with the pet grooming company. The Pet Owner acknowledges that the final grooming results may vary based on the pet’s coat condition, behavior and specific instructions. The Pet Groomer will make every reasonable effort to achieve the desired grooming results. PET OWNER RESPONSBILITIES The Pet Owner certifies that their pet is in good health and has been free from contagious diseases for at least 14 days prior to the appointment. The Pet Owner should ensure that their pet’s vaccinations are up to date, if applicable. The Pet Owner acknowledges that all information provided in the client intake form is accurate. It is the Pet Owner’s responsibility to inform the Pet Groomer of any precious behavioral issues. If the Pet Owner fails to do so, they will be responsible for any medical expenses or damage to the property resulting from their pet’s misbehavior. RABIES By law the new client’s pets or new pets must be up to date on the vaccination. For puppies, that would require a minimum of rabies and all parvo shots. For adult does, that requires a minimum of a rabies shot. A copy of vaccination records must be brought I before any service can occur. CANCELLATION To cancel or alter services without incurring a cancellation fee, the Pet Owner is required to provide notice at least 23 hours prior to the appointment. If a cancellation is made after this period, the Pet Groomer has the right to charge 50% of the grooming fees, which must be paid before further appointments can be made. APPOINTMENT TIME The Pet Owners expected to arrive promptly at the scheduled appointment time. While every effort will be made to accommodate late arrivals, it may result in a shorter grooming session. The Pet Groomer also has the right to cancel or reschedule the appointment of the Pet Owner arrives more that 30 minutes late for their appointment. PAYMENT AND FEES The Pet Owner agrees to pau the Pet Groomer the service fees as outlined in the price list available at the front desk and online. There is no refund on services provided. Payment is due at the time of pick up. The Pet Groomer accepts cash checks, visa, Mastercard and American Express. A $25 fee will be added to the Pet Owners balance if a check is returned. The Pet Owner will not be able to make another appointment until the returned check plus the fee is paid. MATTING In cases of severe matting where removal is not possible without causing stress to your pet, the Pet Groomer may recommend trimming or shearing the affected areas. There may be an additional fee charged for this proves, and the Pet Owner acknowledges that the Pet Groomer will not be held responsible for any irritation, redness or itchiness after grooming due to a matted coat. FLEAS AND TICKS Because of the repaid spread of fleas and ticks in our area, the Pet Groomer asks that your pet be flea and tick free before entering. For minor cases, or if fleas and ticks are found on a pet, the Pet Owner will be notified immediately. The Pet Owner will be given the option to either be charged $15 for a flea and tick bath, or the pet can be quickly picked up. The appointment can be rescheduled after the pet has been treated. If a pet is infested to the point, it is a serious health concern, the Pet Owner will need to pick up the pet immediately so the pet can be taken to the veterinary clinic of their choice. EMERGENCIES AND ACCIDENTS The Pet Owner acknowledges their responsibility to provide accurate and updated contact information and to inform the Pet Groomer of any known health issues or allergies their pet may have. Although caution is taken to avoid any type of injury, there is always a risk of an accident when working with animals, injuries may include cuts, nicks, scratches, quacking of their nails, etc. these are most often caused by wiggling or other movement during grooming. In the event an accident does occur, the Pet Owner will be notified when at pick up if it is not a serious injury. In the event of a medical emergency or accident, every reasonable effort will be made to contact the Pet Owner or their emergency contact immediately. If the Pet Owner or emergency contact cannot be reached in time, the Pet Owner authorizes the Pet Groomer to seek the veterinary care necessary for the pet’s health, safety, and well-being. The Pet Owner shall be responsible for all associated veterinary expenses if their pet is old, in poor health, aggressive, or temperamental. In the event of an accident or incident that results in injury or discomfort t your pet the grooming business will promptly document and report the incident to the Pet Owner. DANGEROUSLY AGGRESSIVE ANIMALS The Pet Groomer reserves the right to refuse or stop service at any time if an animal is dangerously aggressive or is a risk of causing serious or life=threatening injury to themselves, the staff, or other clients. If partial services have been given at time of refusal, the Pet Owner will only be charged for the service that was completed. If severe harm comes to a staff member due to an animal’s aggression the Pet Owner will be charged for the staff members medical expenses, if any harm comes to a dog due to the aggression of another dog, the Pet Owner of said aggressive is liable for the injured dog’s medical expenses. If you have a rescue dog and you know it was a bait dog or severely traumatized, we need to know so we can make appropriate preparations for your fur baby. FORCE MAJEURE Neither party shall be held accountable for unforeseeable circumstances or events beyond their control, including acts of nature, government actions, war, epidemics or other events hindering the contract’s performance. The affected party must promptly notify the other party and make reasonable efforts to mitigate the events effects. SEVERABILITY If any provision of the Agreement is found to be invalid, illegal or unenforceable under applicable law, the invalidity, illegality, or unenforceability of such provision shall not affect the validity or enforceability of the remaining provisions of this Agreement. No modification, amendment, or alteration of this Agreement shall be binding unless it is made in writing and duly executed by both parties. By signing below, the Pet Owner acknowledges and agrees to the terms and conditions stated in this Agreement.
