By booking an appointment with Embody Wellness, clients agree to adhere to the following policies and terms: Clients must cancel or reschedule appointments at least 24 hours before the scheduled start time to avoid a cancellation fee. Complimentary promotional gift cards are subject to the following terms and conditions: * Limit one (1) complimentary gift card per client. * Valid only toward regularly priced aesthetic services unless otherwise approved by management. * Not valid for services offered at cost as in model/training pricing. * Gift cards have no cash value and may not be redeemed for cash or credit. * All treatments are provided solely at the professional discretion of the injector. Presentation of a gift card does not guarantee treatment. Services may be declined if, in the injector’s clinical judgment, treatment is deemed unsafe, inappropriate, or contraindicated. * Gift cards are transferable and may be used by another client in accordance with these terms. Clients are responsible for providing accurate and up-to-date information regarding their health and medical history. Embody Wellness is not liable for any injuries or damages incurred during or as a result of services provided. Clients agree to hold harmless and indemnify Embody Wellness and staff from any claims or liabilities. Any disputes shall be resolved through arbitration. To maintain a safe, respectful, and professional environment for our clients and staff, we reserve the right to refuse or discontinue service at our discretion. By booking an appointment, clients acknowledge that they have read and agree to these policies and terms.
