I understand that it is my responsibility to keep my eyes closed and be still during the entire procedure, until my mink lashes specialist addresses me to open my eyes. I understand that some risks of this procedure maybe but are not limited to Eye redness, swelling of eyelids and irritation. The fumes from the adhesive may cause my eyes to water if I open my eyes. Eyelash extensions are not for everyone. This is a high- maintenance beauty treatment, which requires a gentle care for the lashes to remain in good condition and be long lasting. One the lashes are attached; you must not rub or pull on them. You must not wear any mascara other than that recommended by your technician. You must not use lash curlers. Please read and understand all the following information listed below and sign the form prior to having the service performed. When extensions are applied correctly, they will not harm your natural lashes; each lash extension is adhered to one single natural lash, the extension will grow out naturally, as your own lash matures. The eyelash adhesive is formulated to create a solid bond, specifically for human lashes. The adhesive dries very soft on the lashes, allowing the lashes to stay flexible and bouncy just like your own lashes. I have read the above information and agreed to continue with the eyelash extension service. I understand and agree that if I experience any issues (such as allergic reaction, swelling or irritation) with my lashes that I will contact my technician (Ms. Nevers) within 24 hours of my appointment to have them removed. I will seek medical help using my own expenses & insurance. I understand I will not receive a refund.
