Clients can cancel or reschedule appointments up to 24 hours before start time. $10 late fee will be added if over 9 minutes late. Voluntary Participation: I, the undersigned client, acknowledge that I am voluntarily seeking lash extension services and am fully aware of the nature of the service being provided. 2. Health and Safety: I confirm that I have disclosed any allergies, sensitivities, or medical conditions that might affect the lash extension process. I understand that it is my responsibility to inform the service provider of any changes in my health that may impact the service. 3. Risks and Side Effects: I am aware that while lash extensions are generally safe, they carry potential risks, including but not limited to eye irritation, allergic reactions, and damage to natural lashes. I accept these risks and agree to proceed with the service. 4. Aftercare Instructions: I agree to follow all aftercare instructions provided by the service provider to ensure the longevity and health of the lash extensions. I understand that failure to follow aftercare instructions may affect the results and condition of the lash extensions. 5. Liability Waiver: I hereby release and hold harmless the service provider, their employees, agents, and affiliates from any claims, liabilities, or damages arising from the lash extension service, including any potential adverse effects or dissatisfaction with the results. 6. No Suing Clause: I agree not to pursue any legal action against the service provider for any issues arising from the lash extension service, provided that the service provider has adhered to the agreed-upon procedures and aftercare instructions. 7. Refunds and Touch-Ups: I understand that any concerns or issues with the lash extensions should be addressed within 48 hours of the application. The service provider may offer touch-ups or adjustments in accordance with their policy, but refunds are not provided for dissatisfaction with the results. 8. Client Responsibilities: I agree to communicate openly with the service provider regarding any concerns or issues I may have and to adhere to the policies set forth by the service provider. By booking this appointment, you acknowledge that you have read, understood, and agreed to all terms and conditions outlined in this agreement.
