Policy •Before booking an appointment, please read the policy. you are agreeing to them once you book an appointment with me, thank you. •if you have any symptoms of being sick/ having covid, please let me know before your appointment. •$15 non-refundable booking fee is required at the time of booking. •there are no refunds on any services. •come with clean lashes and a clean face, no makeup, mascara, oils, etc. •no extra guests allowed unless they were approved by me beforehand. •IMPORTANT: inform me of any allergies please and remove contact lenses before your appointment. • a minor fix: contact me within 48 hours after your appointment or you will be charged a touch up fee! ($45) Late/cancellation policy: •same day cancellations and no call no shows will result in a $50 charge, if the fee is not paid then you will no longer be able to book with me. No exceptions •there is a strict 10 minute grace period. *LASH COURSE IMPORTANT INFO: •the course does NOT license you, it is strictly an informational course if you want to learn my techniques and learn from me. It is NOT legally a license . . . By booking, you CONSENT to the following: By booking an appointment with lashed by elma, i consent all of the following, •I consent to the application, removal and/or retouching of eyelash extensions to my natural eyelashes by the eyelash technician listed below ‘Elma Kurtic’ •I understand there are risks associated with the application/removal of eyelash extensions. including, but not limited to eye irritation/ discomfort •I understand that i must remove contact lenses or there will be risk including but not limited to going blind, irritation in the eyes, discomfort. •I agree to follow proper after care instructions given by the lash technician and realize that failure to follow these instructions may result to damage to my natural eyelashes and/or decrease the life of my extensions •’Lashed with Elma’ is not responsible/liable for any irritation /discomfort during the appointment ‘Lashed with Elma’ is not responsible for any improper after care by client I consent that i am the age of 18/ i am under the age of 18 and my parents/guardian has consented to my application I give permission for the release of photos and videos taken of me and my lashes I agree to let ‘lashed with elma’ know of any allergies and or existing medical conditions By booking an appointment with ‘lashed with elma’ i agree and consent to all information listed above, this contract will remain in affect for every appointment booked with Elma Kurtic.
