Client Agreement 

By booking with Irmatherapy Studio, you acknowledge and agree that:

• consent form Manual are required to sign in at first visit and to be updated anytime 

• Your personal information will remain private and confidential for file and records 

• Health history information is required to provide safe and appropriate treatment and must be updated if your condition changes

• By proceeding with treatment, you release "Irmatherapy Studio" from liability related to undisclosed health conditions

• Your health 
information will only be used to support safe and appropriate care

• After Treatment you are advised by Therapist with home self care and suggested to review any contraidications related your your Activities.

• Irmatherapy Studio is not responsible for lost or damaged belongings which is recommended to not use any jewelry or accesories for the treatment Procedures.

• Services are professional, and respectful behavior is required at all times

• Inappropriate suggestion or  behavior will result in immediate termination of service with full payment still required. 

• No companions are allowed in the treatment room, except a parent or guardian for underage clients.

• Payment must be completed before leaving the property

• Clients are responsible for any damages property caused by personal negligence

IRMATHERAPY & STUDIO may cancel or reschedule a confirmed appointment if needed and will notify you as soon as reasonably possible. Please arrive on time; arriving late may shorten your service or require rescheduling. If you need to cancel or reschedule, please do so as early as possible; late cancellations and no-shows may incur a fee.
