Eccentric Bliss Tips & Toes – Nail Services Agreement By booking an appointment with Eccentric Bliss Tips & Toes, you acknowledge and agree to the following terms and conditions. These policies are designed to ensure a safe, respectful, and professional experience for both client and technician. ⸻ 1. Service Agreement By confirming your appointment, you are entering into a service agreement with Eccentric Bliss Tips & Toes for professional nail services. All services are performed by a licensed nail technician in compliance with Michigan state regulations. ⸻ 2. Appointments & Grace Period Clients are expected to arrive on time. A 15-minute grace period is allowed. After 15 minutes, the appointment may be canceled or rescheduled, and your deposit will be forfeited. ⸻ 3. Guest Policy Only one (1) guest may accompany the client during the appointment. Additional guests, including children, are not permitted unless discussed in advance. ⸻ 4. Pricing & Payment • All service pricing is subject to change based on length, design complexity, and materials used. • Accepted payments: Cash, Zelle, Cash App, and Apple Pay. • Checks are not accepted. • A $10 non-refundable deposit is required to secure your appointment. ⸻ 5. Cancellations & Refunds • You may cancel or reschedule up to 24 hours in advance to retain or transfer your deposit. • No-shows or late cancellations result in loss of deposit. • Persistent cancellations or no-shows may lead to a ban from future services. ⸻ 6. Hygiene & Licensing Eccentric Bliss Tips & Toes adheres to the sanitation and safety standards mandated by the Michigan Department of Licensing and Regulatory Affairs (LARA). All tools and workstations are properly disinfected after each use. ⸻ 7. Liability Disclaimer • Eccentric Bliss Tips & Toes is not responsible for allergic reactions, infections, or nail damage caused by improper home care or failure to disclose conditions (e.g., fungus, allergies, injuries). • It is the client’s responsibility to disclose any medical or skin conditions prior to service. ⸻ 8. Confidentiality & Privacy Client information, including contact details and any personal conversations shared during services, will be kept strictly confidential. Photos and videos of nail services may be used for promotional purposes on social media, but clients’ faces and identifying features will never be shown without consent. ⸻ 9. Governing Law This agreement is governed by the laws of the State of Michigan.
