Slay Lashes

By booking an appointment, you agree to the following policies: Deposits: A $50 non-refundable deposit is required upon booking for any NEW lash set only. This deposit will be applied toward the total cost of your lash set. For waxing services, a $10 non-refundable deposit is required for most (but not all) appointments. Cancellations & Rescheduling: Please arrive on time for your appointment. If you are more than 10 minutes late without prior contact, your appointment will be cancelled. No additional cancellation fee will be charged beyond the deposit. To reschedule, please message your lash tech on the social media platform you used to book your appointment. A 24-hour cancellation notice is required for ALL appointments. Multiple cancellations without proper notice may result in refusal of future service. No-shows will not be rescheduled. Guests: For safety and comfort, no extra guests including children are permitted during your appointment. If someone is brought with you, the appointment will be cancelled. Service Guidelines: The address for your appointment will be sent 24 hours in advance. For waxing services, please ensure hair is long enough (approximately the length of a grain of rice). For fills, I acknowledge at least 40% of lash extensions must remain to be considered a fill and anything less will result in the charge of a new set. For foreign fills, I acknowledge that if deemed necessary by the lash technician, my lash set might need to be removed and a fresh new set will be applied. Possible Risks and Adverse Reactions to Eyelash Extensions: I acknowledge that while rare, allergic reactions to lash extension adhesives can occur. Symptoms may include redness, itching, swelling, or irritation in the eye area. I understand that if I experience any adverse reactions, I must contact Stephanie immediately and seek medical attention if necessary. Possible Risks and Adverse Reactions to Waxing: I voluntarily consent to the waxing procedure and understand that individual responses to waxing may vary. I am aware that there may be temporary redness, irritation, or discomfort associated with the procedure. Photography: I hereby grant Slay Lashes permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of Slay Lashes and will not be returned. I hereby irrevocably authorize Slay Lashes to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge Slay Lashes from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I consent to Stephanie/Slay Lashes taking photos or short video clips before, during, or after my service for documentation purposes. These will be stored securely and will only be used for business records, training, or to resolve any payment disputes. Aftercare: I understand that I am required to follow the aftercare in order to maintain the life of the service provided. This agreement will remain in effect for this procedure and all future follow-ups conducted by the certified professional. I understand that this consent agreement is legal and binding. I have read and fully understand all information in this agreement. I consent to the agreement and to the scheduled procedure. I hereby release any and all persons representing this establishment from all demands, claims or damages, actions or cause of actions arising out of the performance of this service. I agree to disclose any allergies that I may have to latex, surgical tapes, etc. I confirm and agree to the terms and conditions set on this consent form and give my permission to engage with the services I scheduled for.
