By booking an appointment, you acknowledge that you have read, understood, and agreed to the following terms. ⸻ FOR FIRST-TIME CLIENTS All intake and consent forms are digital and must be completed prior to your appointment. Once completed, please message me directly with: • Full name • Appointment date & time This allows me to approve your booking and send you directions to my suite. 📱 Phone: 972-513-4383 📸 Instagram: @fatixskin ⸻ RETAINER / DEPOSIT POLICY • A 50% non-refundable retainer is required to secure your appointment. • This retainer goes toward your service total. • Retainers are non-refundable for: • Same-day cancellations • Late cancellations (less than 48 hours) • No-shows • If you reschedule at least 48 hours in advance using the booking link, your retainer will automatically transfer to your new appointment. ⚠️ Failure to reschedule through the booking link within the 48 hour window will result in forfeiture of the retainer. ⸻ CANCELLATIONS & NO-SHOWS • Cancellations made less than 48 hours before your appointment are not eligible for refunds. • No-shows will forfeit their retainer and may be required to prepay future appointments. • Illness, emergencies, or personal circumstances do not override the cancellation policy. ⸻ CHARGEBACKS & DISPUTES By booking, you agree not to dispute or charge back any retainer or service fee that falls within these policies. Any chargeback filed in violation of these policies may result in: • Permanent refusal of future bookings • Responsibility for any dispute or processing fees incurred ⸻ PAYMENT METHODS • Remaining balance is due at the time of service • Accepted forms of payment: • Cash • Tap-to-pay • Card on file ⸻ LATE POLICY • Please arrive on time • A 10-minute grace period is allowed • After 10 minutes: • $10 late fee • $1 per additional minute Excessive lateness may require rescheduling and forfeiture of the retainer. ⸻ APPOINTMENT GUIDELINES • Photos may be taken before and after service for documentation • Extra guests are not permitted unless approved in advance • Same-day appointments during business hours may be available (no extra fee, subject to availability) • Appointments outside of business hours require an additional fee, based on time and service ⸻ CLIENT SATISFACTION Your satisfaction is important to me. If you have concerns about your service, please contact me within a reasonable time frame so we can address them appropriately. 𝐖𝐚𝐱 𝐢𝐧𝐟𝐨: 𝐂𝐨𝐧𝐭𝐫𝐚𝐝𝐢𝐜𝐭𝐢𝐨𝐧𝐬: If you are on any acne treatment such as accutane, most likely you will not be a good candidate for wax services If you have dermatitis, eczema, or any of these conditions such as acne or broken skin, you will not be a good candidate for wax services 𝐁𝐞𝐟𝐨𝐫𝐞 𝐰𝐚𝐱 𝐚𝐩𝐩𝐨𝐢𝐧𝐭𝐦𝐞𝐧𝐭: make sure to grow out your hair for 3 to 4 weeks before getting your wax makes it so much easier to remove the hair flawlessly exfoliate and moisturize the night prior to your wax appointment be sure to wear loose and comfy clothing when coming into your appointment, you want your skin to breathe and relax for the rest of the day 𝐀𝐟𝐭𝐞𝐫𝐜𝐚𝐫𝐞 𝐟𝐨𝐫 𝐰𝐚𝐱𝐢𝐧𝐠: For the first 24 hours after a wax: - Avoid touching the area - Do not exfoliate the area - Do not exercise or swim - Keep the area clean & dry - Avoid exposing the waxed area to sunlight or UV rays - Stay away from makeups, deodorants, perfumes, & soaps - since they can cause irritation After 3 to 10 days after getting a wax: - Gently exfoliate the area a few times a week. Be careful not to over-exfoliate & irritate your skin. After each exfoliation, use a skincare grade moisturizer on the area. - After about 3 days, the skin will begin to close over the hair follicles, gentle exfoliation can prevent those hairs from becoming ingrowns. - schedule your next session with 4-6 weeks after last appointment. Proper aftercare makes following sessions so much easier. *If you have any questions regarding any wax services please feel free to send me a message about.* 𝐁𝐫𝐨𝐰 𝐬𝐞𝐫𝐯𝐢𝐜𝐞𝐬 𝐢𝐧𝐟𝐨: 𝐂𝐨𝐧𝐭𝐫𝐚𝐝𝐢𝐜𝐭𝐢𝐨𝐧𝐬: If you are on any acne treatment such as accutane, most likely you will not be a good candidate for brow laminations, tint & wax services If you have dermatitis, eczema, or any of these conditions such as acne or broken skin, you will not be a good candidate for brow laminations, tint & wax services If you have sparse eyebrows you will not be a good candidate for brow laminations, tint & wax services 𝐁𝐞𝐟𝐨𝐫𝐞 𝐛𝐫𝐨𝐰 𝐚𝐩𝐩𝐨𝐢𝐧𝐭𝐦𝐞𝐧𝐭: Avoid using exfoliants, retinoids, and AHA's on your face for at least 1-2 weeks prior to the appointment. This is because these substances can interfere with the adherence of the wax used in the lamination process. Avoid tweezing, shaving, and trimming your eyebrows at least one week prior to the appointment. Trimming eyebrows before a lamination can result in a choppy brow look. Recommended to come with clean brows free of any makeup, product buildup or hard brow gels- this can affect the proper layering of the brows. 𝐀𝐟𝐭𝐞𝐫𝐜𝐚𝐫𝐞 𝐟𝐨𝐫 𝐛𝐫𝐨𝐰 𝐬𝐞𝐫𝐯𝐢𝐜𝐞𝐬: -Brush your brows daily & moisturize them with aftercare oil - Do not get brows wet for the next 24 hours - Do not apply makeup or receive any other eye treatments for at least 24 hours - Avoid steam, saunas, heat, water or heavy workouts causing sweat for 24 hours - Use a brow conditioner daily - Do not touch your eyebrows after treatment - No self tanning products should be used on the face for 48 hours after the treatment - Avoid prolonged exposure to sunlight or heat to prevent premature fading or discoloration of the tint - Do not apply Retin-A, alpha hydroxy acid or similar skin treating topicals around the brow area for 72 hours - Treat any residual stinging or swelling with a cold compress - Any excess moisture can cause a slight curl or frizz to the brow hair - When you resume washing your face, use an oil free cleaner *If you have any questions regarding any brow services please feel free to send me a message about.* 𝐋𝐚𝐬𝐡 𝐥𝐢𝐟𝐭 & 𝐭𝐢𝐧𝐭 𝐢𝐧𝐟𝐨: 𝐂𝐨𝐧𝐭𝐫𝐚𝐝𝐢𝐜𝐭𝐢𝐨𝐧𝐬: If you have any type of eye infection such as conjunctivitis & blepharitis, you will not be a good candidate for lash lift services If you have had any eye surgery within the past year, you will not be a good candidate for lash lift & tint services 𝐁𝐞𝐟𝐨𝐫𝐞 𝐥𝐚𝐬𝐡 𝐥𝐢𝐟𝐭 & 𝐭𝐢𝐧𝐭 𝐚𝐩𝐩𝐨𝐢𝐧𝐭𝐦𝐞𝐧𝐭: Arrive with no makeup and clean lashes. Avoid wearing waterproof mascara a few days before the appointment as it can leave unwanted residue. Avoid consuming too much caffeine before the appointment as it can cause the eyes to twitch and make the treatment more difficult. If wearing contacts, remove them before the appointment for extra precaution, but they can be put back in after the appointment. 𝐀𝐟𝐭𝐞𝐫𝐜𝐚𝐫𝐞 𝐟𝐨𝐫 𝐥𝐚𝐬𝐡 𝐥𝐢𝐟𝐭 & 𝐭𝐢𝐧𝐭: -Brush your lashes daily & moisturize them with aftercare oil - Avoid getting your lashes wet for 24 hours - No pillow cuddling sleep on your back for the first few days - Avoid steam, saunas, heat, water or heavy workouts causing sweat - Do not have any other facial treatment - Condition your eyelashes twice a day - Be gentle, do not rub or pick at your eyelashes - Be sure to use an oil free make up remover - Wait 48 hours to apply mascara.
